Please complete and return the attached form to the VAL League Chairman (ValleyAL.org) and Participating School Athletic Director in the event of any:

DISQUALIFICATION, PENALTY, OR EJECTION for flagrant, unsportsmanlike conduct, bench fouls or other misconduct of a serious nature committed by anyone associated with a contest you witnessed.

Please file this form within 24 hours of any incident.
